To book or change an appointment you may email us or phone 905-771-8888. We would appreciate 48 hour notice for any cancellations in order to avoid a charge.

In the case of a dental emergency please call our office during regular office hours. After regular office hours you may contact your dentist by email.

Your Dental Insurance
We will submit your dental treatment to your insurance company electronically for you.
We will accept payment from your insurance company but you, as the patient or subscriber are responsible for any portion of treatment that is not covered by your dental insurance. You may bring your insurance booklet into the office and we will be happy to help you understand your coverage. If there are any changes to your insurance coverage, please let us know. Ultimately, you are responsible for payment regardless of coverage and we recommend treatment based on your needs as opposed to your dental insurance coverage. If you are unable to find your booklet, you may call your insurance company to get the information you require. The insurance company is obligated to give you this information. As a dental office we cannot get this information for you without you present in the office. Please download this form and complete it prior to your appointment. You should have your insurance policy number and information ready when you call the company,
